Health Screenings

All students entering PreK, kindergarten, Grade 1, Grade 3 and Grade 5 must have a health examination performed by a New York state licensed physician, physician assistant or nurse practitioner. The examination must be completed on the NYSED Student Health Examination Form.

A dental certificate which states your child has been seen by a dentist or dental hygienist is also asked for at this time. The school will provide you with a list of dentists and registered dental hygienists who offer dental services on a free or reduced cost basis if you contact your school’s nurse.

Vision, hearing and screenings will be completed during the school year for new students in Pre-K, Kindergarten and Grades 1, 3 and 5. All new students will receive color perception screening.

  • A copy of the health examination must be provided to the school within 30 days from when your child first starts school, and when your child starts kindergarten, Grade 1, Grade 3 and Grade 5, Grade 7.
  • If your child has an appointment for an exam during the school year that is after the first 30 days of school, have your physician’s office send a confirmation of this appointment to Ms. D’Ambro in the Health Office.

Students entering Grades 7 and 8 must have the meningococcal vaccine. Without it, they can’t start school. 

Vision screenings (distance or near acuity) will be completed during the school year for new students and students in Pre-K, Kindergarten and Grades 1, 3, 5 and 7.

Hearing screenings will be completed for all new students and students in Pre-K, Kindergarten and Grades 1, 3, 5 and 7.

Scoliosis screenings will be completed for all girls in Grade 5 and 7.
